INDIANA IMMUNIZATION REQUIREMENTS

2010-11

Kindergarten

• 5 doses DTP/DtaP, DT, or Td

• 4 doses polio

• 2 doses measles-containing vaccine given after 1st birthday

• 1 dose rubella vaccine given after 1st birthday

• 2 doses mumps given after 1st birthday

• 3 doses Hepatitis B

• 2 doses of varicella vaccine or doctor verification of chickenpox

Grades 1-5

• 5 doses of DTP/DTaP, DT, or TD

• 4 doses polio

• 2 doses measles-containing vaccine given after 1st birthday

• 1 dose rubella vaccine given after 1st birthday

• 2 doses mumps vaccine given after 1st birthday

• 3 doses Hepatitis B

• 1 dose of varicella vaccine or history of chickenpox disease

Grades 6-12

• 5 doses of DTP/DTaP, DT or TD

• 1 dose of Tdap

• 4 doses of polio

• 2 doses of measles-containing vaccine given after 1st birthday

• 1 dose rubella vaccine given after 1st birthday

• 2 doses mumps vaccine given after 1st birthday

• 3 doses of Hepatitis B vaccine

• 2 doses of varicella vaccine or history of chickenpox disease

• 1 dose of meningitis vaccine

Preschool (associated with school)

• 4 doses of DTP/DTaP, DT, or TD

• 3 doses polio

• 1 dose of measles-containing vaccine given after 1st birthday

• 1 dose rubella vaccine given after 1st birthday

• 1 dose mumps vaccine given after 1st birthday

• 3 doses of Hepatitis B vaccine

• 2 doses of varicells vaccine or medical verification of chickenpox disease

The only wavers granted in Indiana are medical or religious.  These must be given yearly in writing.
